Three phase motors in general a very simple machines but can get very complicated when something is wrong, in an ideal setting a 3 phase motor will never fail, the 2 main reasons for failure include power supply faults and mechanical failure of the bearings/housings for bearings, some manufacturers even state that given constant power and regular maintenance their motors will last longer than the machine it is running.
